Complaint Investigation Report
(2) staff interviewed stated that they were having personal conversation on Tuesday evening 01/20/2026 when C1 overheard part of it and misunderstood the whole conversation. According to (3) of (5) staff interviewed, another incident happened the following day, 01/21/2026 when the majority of the staff and clients, including S2 and C1, were watching a music video while C4 was dancing. In one scene, two ladies were seen kissing and dancing . C1 thought the staff hated lesbians when S2 then changed the channel . All staff interviewed indicated that C1 frequently seeks to get attention by making false claims or accusing others of misconduct . Staff also indicated that C1 is an attention seeker who gets jealous when the staff pays attention to other clients. Documents reviewed revealed that C1 has a history of fabricating stories and making claims in order to get people's attention. (2) clients were out in the day program, (1) was in the hospital, and were not interviewed. LPA reached out to the Regional Center and they confirmed that they did not receive this report and were not conducting an investigation. Therefore there was insufficient evidence to corroborate with this allegation.
Based on statements and interviews conducted with clients and staff as well as reviewed files and documentation, there was not enough supportive evidence to corroborate the allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
