Facility Evaluation Report

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Case Management Deficiencies in conjunction with a complaint visit (Complaint Control # 28-AS-20250903143605 ). The purpose of this visit is to issue a deficiency that was observed by LPA that is not part of the complaint allegation. The reason for the Case Management - Deficiencies was explained to Administrator Ronnie Chua and Hannah Ortega, Interim Administrator.

During the complaint visit on 09/09/2025, LPA was unable to find any incident reports about the (2) incidents that happened on 09/03/2025 and 09/04/2025 between Client #1 (C1) and Client #2 (C2) involving aggressive behaviors/physical altercation, aggression to staff and property destruction. Interview with the Administrator confirmed during the interview that no incident reports were sent to Community Care Licensing (CCL) since the Regional Center advised them that it is not necessary to notify CCL.

Deficiency is noted on LIC 809D. Exit interview, a copy of this report and Appeals Rights were provided to Ronnie Chua, Administrator and Hannah Ortega, Interim Administrator.
