Complaint Investigation Report

Allegation: Staff threatened client. It has been alleged that staff threatened to hit a client. Staff interviews revealed that staff do not threaten clients (including hitting clients). Interviewed staff indicated that C-4 has a history of fabricating stories which is being monitored and recorded by staff. Interviewed staff indicated that they have not witnessed any staff threatening clients (including hitting clients). Client interviews revealed that staff do not threaten clients. Interviewed clients indicated that they have not witnessed any staff threatening clients (including S-5 and C-4). Client interviews revealed that staff are nice and respectful and did not have any concerns regarding this matter. Interviews and reviewed documentation do not corroborate this allegation.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
