Facility Evaluation Report
Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit. LPA was met by Sandy Alvarez, LVN/House Manager and explained the purpose of today's visit. Shortly after, Matthew Riddell, Project Manager and Aaron Yarbrough, Interim Administrator arrived and assisted with the inspection. The facility is approved for (4) non ambulatory developmentally disabled adults, age range 18-59, approved for delayed egress. . The services are provided by San Gabriel Pomona Regional Center. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Staff are adhering to infection control requirements.
Operational Requirements: A fire clearance is in place. Fire drill is conducted every (6) months. Last fire drill was held on 08/06/2025. Staff cannot provide proof of Surety bond insurance.
Physical Plant & Environment Safety: This facility is a single story home consists of (5) bedrooms, of which (4) are client bedrooms and (1) office, (4) bathrooms, laundry area, kitchen, dining room, living room w/covered fireplace, activity/tv room, attached garage and a backyard with shaded area with seating. Currently, there are (4) clients residing in the home. During the visit, the entire house is being painted outside and LPA observed (1) worker outside. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Laundry area is next to the kitchen. There are (4) fire extinguishers which were last serviced on 05/13/2025. Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Hot water supply measured 108.6 deg F in bathroom #1 and 109 deg F in bathroom #2 and 108.5 deg F in bathroom #3. Exit doors are free of any obstruction and there are no pools or large bodies of water. Backyard was inspected and has a shaded area and sitting area. *****CONTINUED ON LIC 809C******
