Facility Evaluation Report
Licensing Program Analyst (LPA), Sandra Urena arrived unannounced for a case management-other visit. The LPA was greeted by staff, and staff called the Administrator on the phone. The LPA spoke with the Administrator Jheria Frazier, on the phone and explained the reason for the visit. Per the Administrator they attempted to collect the letter from the post office, however the letter would not be released to the Administrator because the letter was addressed to the Licensee. The Licensee has been unavailable due to personal reasons, consequently the letter was not collected. The LPA recommended to the Administrator that in the future, they should contact their assigned LPA if any questions or concerns arise.
On 06/13/2025, Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced visit to issue civil penalties. The Department issued a civil penalty per Health and Safety Code § 1548(f)(1)(A) for a violation that the Department constituted as serious bodily injury in the amount of $10,000. However, since an immediate civil penalty of $500 was previously issued on May 13, 2022, the amount of the civil penalty issued was $9,500.
Today, 12/22/2025 LPA Urena is hand delivering the Invoice of Civil Penalties Due (LIC 422), which was returned to the Woodland Hills North Regional Office (WHNRO) on October 8 th , 2025, as unclaimed mail.
Exit interview was conducted on the phone with the Administrator, and a designated facility staff signed off on the report. A copy of the report was issued.
