Facility Evaluation Report
On 10/29/25 Licensing Program Analyst (LPA) M. Garza arrived at the facility unannounced to complete a case management visit. This case management visit is being conducted to address issues that were observed during investigation of complaint #24-AS-20250710102232. LPA met with Administrator, Pablo Armigo, explained reason for visit, and was permitted entry into facility. No residents were present during today’s visit.
During investigation into the complaint above the following issues were observed: In reviewing SIRs for R1, it was noted that on 05/13/25, 05/17/25, 05/26/25, there were incidents that occurred in the community with staff’s observation of R1’s attention directed at younger aged children for too long. Staff were able to engage R1’s attention to them and leave the area without incident. No deficiencies cited for these SIRs.
SIRs were reviewed for R2, the following was observed: On 12/11/24 R1 reported to staff that R2 was crossing boundaries, trying to touch them and attempts to sexually assault them. R1 was distraught with R2’s behavior. PD was contacted and a report was made. R2 was issued a court date for case #M25906037. A diversion plan was proposed for “Misdemeanor sexual battery by restraint” but does not address if the behavior continues.
