The inspector’s account

On 4/7/26 Licensing Program Analyst arrived at the facility to complete an unannounced case management visit. LPA met with Direct Support Professional, Jaqueline Neria, explained reason for visit and was permitted entry into the facility. Licensee, Ralph Ohannesian and Administrator, Veronica Matthew arrived a some time later. LPA completed a tour of the facility. Residents were not present during todays visit.

This case management visit is being completed for incident reports that were received by the Department.

1) Incident report received for an incident occurring January 21, 2026. S1 was observed on the outside camera surveillance and audio being verbally aggressive with R1 while R1 was changing cat food/water. In the video surveillance S1 was not physical with R1. S1 was provided a write up per facility policy.

2) Incident report received for an incident occurring January 25, 2026. S1 was observed on camera surveillance in the dining area during lunch. S1 was observed to be grabbing bowls and plates from the residents in a "rushing" manner while residents were trying to complete their meals. S1 was observed to walk over to R2 hitting them 3 times in the back. S1 was seen pushing R2 in the shoulder towards a un-monitored area when they got up from the table.

S1 was provided counseling's and written notices. Since February 1, 2026, S1 did not return to work and is no longer working at the facility.

3) Incident report received for an incident occurring March 29, 2026. R3 had a fall that resulted in an injury to the forehead. R3 was provided first aid but Emergency Services was not contacted.

At this time the case management visit requires further investigation. No deficiencies cited during todays visit.

Exit interview completed with Licensee and Administrator. A copy of this report provided.
