On 7/10/2026 Licensing Program Analyst (LPA) K.Kaur arrived at the facility to complete an unannounced annual visit. LPA met Case Managers Robert Perez and Maylen Romos and explained reason for visit and permitted entry into the facility. Program Manager, Kelly Wheeler, was not available to meet.
After review of facility records its was observed an annual inspection was conducted on 05/21/2026. No deficiencies cited during inspection
