On 03/07/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the clients in care. Licensee/Administrator I’isha Cooley was called. LPA spoke with Licensee/Administrator via telephone.
LPA was informed Licensee via telephone, that excluded staff member is not associated with facility and have been taken off schedule since September 2024.
No deficiency was observed. A copy of this report was emailed to Licensee per request. Signed report on file. Exit Interview.
