Licensing Program Analyst (LPA) M Vega arrived unannounced to conduct a Case Management visit on 05/06/2025. LPA met with (Staff 1) S1 and stated the purpose of the visit. S1 contacted administrator, administrator was not able to attend, however gave permission for staff (S1) to sign report.
LPA arrived at facility to conduct a case management visit, conducted an interview with Clients and Staff.
