Licensing Program Analyst (LPA) M Vega arrived unannounced to conduct a Case Management visit on 05/06/2025. LPA met with facility Direct Care Support Staff Desiree Sanchez and stated the purpose of the visit. Licensee I'Isha Cooley was contacted by phone and was unable to attend.
LPA arrived at facility to conduct a case management visit, conducted an interview with Clients and Staff.
LPA requested documents from licensee LIC 625, Training received as well.
No deficiencies were observed and cited. Exit interview held with Direct Care Support Staff Desiree, A Copy of report given and emailed to I'Isha Cooley per licensee request.
