The inspector’s account

On 11/20/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual

inspection. LPA introduced self, stated the purpose of the visit and met with Case manager Jessica Figueroa. Administrator Andrina Thompson-Fernandez was called and arrived shortly.

LPA completed a tour of the facility with Case Manager and Administrator. 24 clients were present during the inspection of the activity rooms. LPA observed medications in locked case manager’s office. MAR was reviewed and medications were checked. Facility is maintained at a comfortable temperature and no passageway obstructions or fire hazards were observed. Fire extinguishers throughout the facility were observed with served date: 12/11/25. Last emergency drill conducted on 10/29/25. Transportation is available and provided for clients. Kitchen was toured. Clients bring their own lunch to program and store them in the refrigerator. Refrigerator temperature observed maintained at 40 degrees F and freezer at -8 degrees F. Client locked lockers were observed throughout activities room. Client restrooms were tour, observed to be clean, and operational. Hot water temperature was 107.2 degrees in front bathroom, 105.1 degrees in bathroom 1, 105 degrees in bathroom 2, and 105.5 in bathroom 3. Washer and dryer observed located in bathroom 3. Washer was observed functioning during visit. Chemicals and cleaning supplies were observed locked in the chemical supply closet. Cleaning chemical bottles were observed unlock in small cabinet next to staff break room. Water fountain observed in good condition. A sample of client and staff files were also reviewed to have the required documents.

A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents are requested and to be submitted to Fresno CCL by: 11/26/25. The following updated forms were requested: Lic 308, Lic 500, Lic 610D, and Lic 9020. A copy of this report and appeal rights were provided to the Administrator, whose signature confirms receipt of this report.
