Facility Evaluation Report
On 07/21/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct required Annual visit. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee/Administrator Nicole Alexander. Three clients are present during inspection.
The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Upstairs area and two down stair rooms are designated for staff live in. Fire extinguisher was observed with a purchase date of 12/17/25.
Fire drill last completed: 07/17/26. Medications observed kept locked in hall closet. An adequate supply of perishable and non-perishable food was observed. Temperature maintained for the refrigerator at 37 degree F, freezer at -1 degree F, and stand up freezer at -8 degree F. Sharps and chemicals observed locked in laundry cabinet. All bedrooms were observed to have the required furnishings and adequate lighting.
Hot water temperature was tested at 107.2 degrees F in bathroom 1 and 106.5 degrees in bathroom 2. First aid kit observed with all required items. Outside of the facility toured and observed to be free of debris. Outdoor seatings observed available for the clients. The side gate observed self-closing.
Carbon monoxide and smoke detectors observed operational during inspection. All clients and sample staff files reviewed to have all required documents.
Exit Interview conducted. LPA received copies of Lic 308, Lic 500, Lic 610D, and Lic 9020. A copy of this report was provided to Licensee, whose signature on this form confirms receipt of this report.
