Facility Evaluation Report
On 12/19/25, Licensing Program Analyst (LPA) M. Yang arrived for unannounced visit discussed case
management visit. LPA introduced self, stated the purpose of the visit, and met with Administrator/Licensee Nicole Alexander.
The department conducted interview and toured the facility. LPA observed a lock latch attached onto the outside of the stand-up deep freezer. Lock latched was observed unlatched and unlock. Food was observed in the stand-up deep freezer unlocked and accessible to clients. LPA discussed with Licensee the rights of clients accessible to food.
An exit interview was conducted. A copy of this report was provided to Licensee, whose signature on this
