Facility Evaluation Report

On 09/03/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct required Annual visit. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee/Administrator Nicole Alexander. All four clients are present during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Upstairs area and two down stair rooms is designated for staff live in.

Medications observed kept locked in hall closet. Medications were checked and MARs were reviewed. Fire extinguisher was observed with a purchase date of 12/12/24. Fire drill last completed: 08/3/25. An adequate supply of perishable and non-perishable food was observed. Temperature maintained for refrigerator at 37 degrees F and freezer at -3 degrees F. Sharps and chemicals observed locked in laundry cabinet. Washer and dryer functional and operational during inspection. Cleaning supplies and chemicals stored and locked in laundry room cabinet. All bedrooms were observed to have the required furnishings and adequate lighting. Hot water temperature was tested at 112.6 degrees F in bathroom 1 and 120 degrees in bathroom 2. Outside of the facility toured and observed to be free of debris. The side gate observed self-closing. All clients and sample staff files reviewed to have all required documents. Carbon monoxide and smoke detectors observed operational during inspection.

Forms requested: Lic 308, Lic 402, Lic 500, Lic 610D, Lic 9020, and current Administrator Certificate. A copy of this report was provided to Licensee, whose signature on this form confirms receipt of this report.
