Facility Evaluation Report
On 1/23/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. LPA introduced self, stated purpose of visit and allowed entrance by Administrator. Yolanda Giudici.
Currently, four (4) residents in care. All residents arrived back from day program during facility inspection. Residents attend day program Monday - Friday, 9:00 am - 2:00 pm.
Facility tour conducted indoors and outdoors with Administrator. Facility observed to be well lit, odor free, indoor thermostat had a indoor reading of 69 degrees F. All common areas have adequate seating available for residents in care. Resident bedrooms have required furnishings, linens observed to be clean and in good repair. Resident bathrooms toured, water temperature measured at 114 degrees. Kitchen tour conducted, a 2-day supply of perishable and a 7-day supply of non-perishable food present and properly stored. All knives are stored and secured in locked box in kitchen cabinet above oven.
All cleaning supplies are stored in a locked storage closet on the back patio. Smoke detectors and carbon monoxide detectors tested and observed operational during inspection. Last fire drill conducted 1/10/2026. Fire extinguisher present and has a service date of 12/08/2025.
Backyard toured, exits open free of obstruction. Sheds are locked, secured, and inaccessible to residents.
Resident and staff files reviewed. Administrator to submit updated LIC 308, LIC 500, and LIC 610 to Fresno Regional office no later than 2/06/2026.
No deficiencies observed during inspection. Exit interview conducted and a copy of report provided for facility records.
