01/06/2026 and conducted by Evaluator Brianna Miranda
COMPLAINT CONTROL NUMBER: 24-AS-20260106171638
Staff did not seek medical attention for a resident in care.
On April 28, 2026, Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to deliver the findings for the allegations listed above. LPA met with Manager I Thai Nguyen.
Regarding the allegation: Staff did not seek medical attention for a resident in care. LPA conducted multiple interviews. LPA was informed R1 was seen in the clinic and sent for x-rays and an ultrasound. Interviewees did not state there was concern for R1's care or treatment.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Exit interview was conducted and a copy of this report LIC9099 was provided to Manager I Thai Nguyen.
