Facility Evaluation Report

An Annual visit was conducted by Licensing Program Analyst (LPA) K. McClurg. LPA met with Director of Adult Program Services (Dir) Kelly Cunningham. LPA stated purpose of visit & was allowed to proceed.

Physical plant toured. Classrooms sufficiently furnished with adequate lighting. Passageways, including hallways, interior & exterior observed to be clear of obstructions.

Sufficient supplies for available for activities. Students have their own lockers. There are sufficient restrooms available. Quiet room available. Hot water measured at 120 degrees F. Facility appeared to be clean with no unpleasant odors. Sufficient supplies of PPE. Medications Centrally Stored in a locked area inaccessible to students.

Smoke detectors operational. Carbon Monoxide Detectors operational. Fire extinguisher service date: 3/9/23.
