Facility Evaluation Report

Licensing Program Analyst (LPA) Katie Brown arrived to conduct the Annual Inspection. LPA met with and explained the reason for the visit with Administrator (AD) Pa Vue. The Day Program currently does not have participants and is not open yet.

During this visit, LPA toured the Day Program inside and out with AD. LPA observed required postings. The large common areas contain tables and chairs and furnishings in good condition with lighting and clear walkways throughout. The Bathroom is clean and contained required items and running hot water as required. Disinfectants and cleaning supplies are locked and inaccessible in Ads office. LPA observed gloves, paper goods and bathroom products which were stored properly and available for use. The kitchen was clean and will be utilized for food preparation and staff break room. An Emergency Kit is stored in the kitchen containing required First Aid items. The quiet room contains comfortable furniture and will be used as an isolation room as well when needed. There is a covered common area outside where participants will be able to congregate. Fire Extinguishers were serviced 6/30/25 by Valley Fire. Emergency Disaster and Infection Control requirements were reviewed. Staff files were reviewed. LPA reviewed medication and training procedures with AD.

There were no citations during this inspection.

An exit interview was conducted and a copy of this report was signed and provided

LPA requested the following updated forms faxed to CCLD by: 6/30/26: Designation of Facility Responsibility (Lic308), Administrative Organization (Lic309), Personnel Report (LIC 500), Proof of current Liability Coverage.
