The inspector’s account

Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct a Health & Safety Inspection in conjunction with an initial complaint visit (Control Number 24-AS-20250508153500 ). LPA explained the reason for the inspection with Program Director (PD) Maria Farias.

LPA, Case Manager (CM) Vanessa Salazar and PD toured the Day Program (DP). Program Participants were observed in 2 separate rooms enjoying lunch. There were 5 groups of Participants out of the facility on Community outings who would return to program around 1PM.

LPA observed multiple common areas during the tour including a computer room, gym, movie room, program kitchen and computer room. Program restrooms were clean and contained required items with faucets delivering hot water. Program and staff break room kitchens were found to be clean with no accessible sharp or knife items. Fire extinguishers were found to be charged and were serviced 1/16/25 by Jorgensen Fire Co.

The Day program grounds were clean. Furnishings were properly placed and in good repair. The facility maintained good lighting. Walkways and doorways were unobstructed throughout. Program Participants have their own lockers with locks to store personal items. The Program does not assist Participants with routine medications.

There were no Health & Safety Concerns observed during this visit. An exit interview was conducted and a copy of this report was signed and provided to PD.
