On 12/16/2025 Licensing Program Analysts (LPAs) B. Miranda & J. Duarte arrived at the facility unannounced to conduct a case management regarding a prior employee. LPAs met with Administrator Anthony Beasley.
Administrator Anthony stated S1 has not worked at the facility since 2023.
No action taken. Administrator is aware S1 is not allowed to work at the facility.
