Facility Evaluation Report
On 4/15/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. LPA allowed entrance by staff, LPA spoke with licensees Dennis and Robin Patton to advise of inspection visit.
Currently, three (3) residents in care. All residents were preparing to leave for day program at start of inspection. Residents attend day program Monday-Friday from 8:30 AM - 2:30 PM.
Tour of the facility conducted with staff. All common areas of facility have adequate seating and lighting for residents in care. Resident bedrooms observed to be clean and have all required furnishings available. Resident bathroom observed to have slip-resistant mats in the shower and fixtures to be operation. Hot water measured at 112 degrees F. Kitchen toured, knives observed to be locked and secured. Facility has a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. All cleaning supplies observed to be locked and secured. Medications are locked and centrally stored in the staff room, first aid kit present with required items.
All exits and passageways are free of obstruction. Fire extinguisher has a purchase date of 3/09/2026. Smoke detectors and carbon monoxide detectors present and observed operational at time of inspection. Resident and staff files reviewed.
Outside of facility toured, no hazards observed. Guest house in the back yard is utilized for storage and inaccessible to residents in care. Emergency exit free of obstruction.
No deficiencies cited during inspection. Exit interview conducted with staff and licensees via telephone. A copy of this report provided for facility records.
