Facility Evaluation Report
On 11/5/2025 Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA was greeted by staff Shranise Fraklin and allowed entrance into the facility. Administrator David Opoku-Ababio was contact and arrived shortly after.
LPA observed the facility to be clean, odor free, and clutter free. LPA did not observe obstructions of any exits. Smoke alarm and carbon monoxide were tested and are in working order. Water temperature was checked in the kitchen and read at 110.8 degrees Fahrenheit.
LPA observed resident bedrooms which were properly furnished with adequate lighting. LPA observed chemicals, knives, and medications to be locked an inaccessible to residents.
LPA observed a small amount of canned foods which is not sufficient enough to be 7 days worth of non-perishable food items. Facility did not have an infection control plan completed. LPA did not observe a physician's script on file for resident's PRNs. LPA observed Centrally stored log for medications to not be completed properly for all resident's medications. Deficiencies were cited under Title 22.
Exit interview was conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Administrator David.
