The inspector’s account

On 5/19/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA arrived, introduced self, stated purpose of visit and allowed entrance by Life Skills Trainer (LST). Administrator, Adan Rubalcava contacted by telephone and arrived a short time later to continue inspection visit with LPA.

Facility has 6 private bedrooms and 3 bathrooms. Facility observed to be clean and a comfortable temperature. All resident bedrooms are private, all resident bedrooms observed to have required furnishings available. Resident bathrooms observed to have grab bars and shower chairs available for residents, Hot water measured between 115 degrees F in the bathroom between bedrooms 1-4 and 112 degrees F in the bathroom near bedrooms 5-6. All common areas of observed to have adequate seating for residents in care. Kitchen toured, facility observed to have a 2-day supply of perishable and a 7-day supply of non-perishable food available. All knives are locked and secured. Laundry room observed to have a locking cabinet for chemicals and additional locking cabinets for supplies. Medications are locked and secured in medication room.

Carbon monoxide detectors present and observed operational at time of inspection. Fire extinguishers present with a service date of 8/05/2025. Facility is equipped with pull station and sprinklers through out and last inspected on 8/01/2025 by RLH Reliable Fire and Security Solution.

Exterior of facility toured, all passageways open free of obstruction. Facility has seating available outdoors for residents. Facility is equipped with a generator and propane tank. There is a shed that is locked and inaccessible to residents. Garage is utilized for physical therapy with treadmill, stationary bike, stairs, and other rehabilitation equipment.

No deficiencies cited. Exit interview conducted and a copy of report provided for facility records.
