COMPLAINT CONTROL NUMBER: 24-AS-20241106151130
Staff do not treat residents with dignity or respect
Licensing Program Analyst (LPA) Katie Brown arrived at the McWealth Administrative Office (1616 W. Shaw Fresno 93711). Administrator (AD) Josh McWealth was unable to attend. Vanessa Valencia was authorized to meet with LPA. LPA explained the reason for the visit is to deliver complaint findings.
The Department investigated the allegation above. Incident Report and interview conducted confirm that video survelliance was reviewed which revealed S2 attempted to prevent R1 from exiting the bedroom. Survelliance also revealed that S2 was disrespectful in commmunication with R1, by yelling at residents. Once discovered, S2 was terminated. Based on interview and Incident Report submitted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
A deficiency is being cited in accordance with California Code of Regulations on the attached LIC 9099-D under the section Personal Rights.
An exit interview was conducted. A copy of this report and Appeal Rights were emailed to AD.
