Facility Evaluation Report
Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility and was greeted by facility staff. Staff called Assistant Administrator Maria Salvacion , who arrived shortly after to assist in the completion of this inspection.
LPA was given binders containing the Emergency Disaster Plan and record of Fire Drills. The last fire drill was conducted on 1/15/2025. The Emergency Disaster Plan was up to date and reviewed 2/6/2025. A binder containing all staff CPR certificates was reviewed, all staff are current.
LPA toured the home with staff. Water temp in resident bathroom tested at 116.8 degrees F. Fire extinguisher was observed and last service was 2/5/2025. Smoke alarms and carbon monoxide detectors were tested and verified operable. Home thermostat was set at 69 degrees F.
A linen closet in the hallway stored multiple sets of bedding and blankets for all residents. Personal hygiene items are stored in a locked closet in the laundry room. All three bedrooms observed had beds, seating, lighting and plenty of closet and dresser space for all their belongings. Each client had a separate locked box beside their bed to store their own personal items if they choose to do so.
Both perishable and non-perishable food was in ample supply.
Medications were stored in a locked cabinet in the kitchen area. A binder with a medication log and a list of centrally stored medications was reviewed. Knives and scissors were stored in a locked cabinet under the kitchen sink.
Resident and staff files were reviewed and found to be in order.
LPA received the following documents in the CCL Fresno RO prior to this visit: LIC308, LIC400, LIC9020, LIC500, LIC402, and LIC610D.
No deficiencies cited on this day. Administrator's signature on this form confirms receipt.
