On 11/13/2025, Licensing Program Analyst (LPA) M. Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA was not able to access facility. knocked on door and there was no response.
LPA attempted communication via phone as well to facility number and secondary number and received no response from administrator. Received a call back from administrator, administrator stated that could not attend visit.
Administrator was not available to sign form.
