Facility Evaluation Report
On 5/30/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, stated purpose of visit, and allowed entrance by Direct Support Professional (DSP).Administrator, Aaron Gallegos contact by telephone and was not available to conduct facility inspection. Telephone authorization provided to conduct facility inspection with staff and sign report
Facility tour conducted with DSP. Facility is a 4 bedroom, and 1 bathroom house. There are four private bedrooms, Currently, there are four residents in care. Three residents were present during inspection. Facility observed to clean, odor free, well lit, and a comfortable temperature. All common areas of the facility observed to have adequate seating for residents. Resident bedrooms toured, bedrooms observed to have required furnishings. Additional linen observed in cabinets in laundry area. Resident bathroom toured, fixtures observed operational at time of inspection. Water temperature measured at 112 degrees F. Kitchen toured, Facility observed to have a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Sharps are locked and secured with medication in closet in office.
Outside of facility toured. Backyard of facility is surrounded by perimeter fence. There is a patio table with umbrella for shad if outdoors. Storage shed in back yard is locked, secured, and inaccessible to residents. No hazards observed
Carbon monoxide detectors and smoke detectors tested during inspection and observed operational. Fire extinguisher present with a service date of 2/17/2025.
Due to Administrator being unavailable, LPA will return to complete inspection tool and review staff and resident files.
