On 10/02/2025, Licensing Program Analysts (LPAs) M. Medina and L. Salazar arrived to conduct an unannounced Annual Required Inspection.
LPA's made contact with adult female, who stated they had been residing in the facility for last 2 months so it remained occupied. It was clarified that there were no residents in the facility.
LPA Medina provided business card, however LPA's did not enter to conduct inspection.
