Facility Evaluation Report

An unannounced visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. McClurg. LPA met with Administrator (Admin) Eric Stoner. LPA introduced self, stated purpose of visit, & was allowed to proceed with visit.

Physical plant toured. Classrooms sufficiently furnished with adequate lighting. Passageways, including hallways, interior & exterior observed to be clear of obstructions. Sufficient number of washroom to client ratio. Bathroom sink temperatures set to fixed temperature for hand washing. Sufficient supplies available for activities. Quiet room available. Facility appeared to be clean with no unpleasant odors. Interior & exterior passageways observed to be clear of clutter &/or obstruction Sufficient supplies of PPE. Smoke detectors operational. Carbon Monoxide Detectors operational. Fire extinguisher service date: 1/16/25. Smoke & carbon monoxide serviced & tested by outside agency. Date of last detector testing: 7/12/24. Date of last emergency drill: 12/10/2024.

Facility records reviewed. Per Admin, ADP provides transportation for approximately 95% of consumers. Transportation logs maintained on all vehicles, including scheduled routine servicing. Consumer & staff records reviewed. Medications reviewed. Medications observed to be organized & stored in a safe manner inaccessible to non-authorized persons. Centrally Stored & Destruction Record (CSMDR) & Medication Administration Records (MARs) reviewed & observed to be appropriately maintained.
