STEPHANIE'S HOUSE A HOME FOR SPECIAL PEOPLE

Staff transported residents in an unsafe manner

Staff did not provide appropriate supervision to residents

Staff did not safeguard resident's medication

Licensing Program Analyst (LPA) K. Kaur arrived at the facility for a subsequent visit to deliver findings. LPA was allowed entry by staff Marissa Cintora. Administrator Elsa Snyder was contacted and arrived a short while later. LPA explained the purpose of the visit and reviewed the elements of the allegations. LPA delivered the following complaint investigation findings.

The Department investigated the allegations listed above. Based on interviews conducted with staff and residents; staff have not spoken inappropriately to residents or transported residents unsafely. Residents’ clothing is thrown away only by the residents or by resident's request. No incident of resident being forced to eat was mentioned or documented. Based on records review and observations, residents’ medication is locked and administered per orders.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore these allegations are UNSUBSTANTIATED.
