Staff practice unusual forms of punishment with clients in care
Staff does not ensure medication is dispensed as prescribed.
Licensing Program Analyst (LPA) K. Kaur arrived at the facility for a subsequent visit to deliver findings. LPA was allowed entry by Staff Lorena Salinas. Administrator Elsa Snyder was contacted and unavailable to meet. LPA explained the purpose of the visit and reviewed the elements of the allegations. LPA delivered the following complaint investigation findings.
The Department investigated the allegations listed above. Based on interviews conducted with staff and residents, staff do not punish the residents. Medication review was conducted and medication is dispensed as ordered by physician. Based on observation and interviews facility is operating within ratio limits.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore these allegations are UNSUBSTANTIATED.
