Facility Evaluation Report
On 5/8/2026, Licensing Program Analyst (LPA) K.Kaur arrived unannounced for complaint inspection and conducted a case management in conjunction. LPA was allowed entry by Staff Lorena Salinas. Administrator was unavailable to meet. LPA introduced self and explained the reason for the visit.
During the course of the complaint investigation, the Department discovered while residents were being transported a vehicle stopped abruptly in front of the facility vehicle transporting residents. Staff had to brake suddenly and serve to avoid hitting the other vehicle. Facility never submitted a incident report to Licensing.
Deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6.
An exit interview was conducted with Administrator including discussing the plan of corrections. Report signed on-site. Printed copy provided with 809D page and appeal rights.
