Facility Evaluation Report
On 07/10/2026 Licensing Program Analyst (LPA) M Vega arrived at the facility for an unannounced Case Management – Health and Safety Check visit. LPA met with staff, explained reason for visit and was permitted entry into the facility. Administrator was not able to attend visit, Direct Support Professional - Rebecca Wilson was able to sign the report.
LPA investigating potential Air Conditioner(A/C) not working. LPA toured facility, at the furthest back thermostat the temperature read 75 Degrees F and LPA thermometer read at 71.7 Degrees F. LPA read temperature at Thermostat near bathroom/dining area at 69 Degrees F and LPA thermometer read at 71.4 Degrees F. LPA took temperature in kitchen with thermometer and it read at 71.7 Degrees F.
No deficiencies issued during this inspection. An exit interview was conducted with the Direct Support Professional - Rebecca Wilson.
