The inspector’s account

On 1/20/23 at 10:15 a.m. Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced for an annual inspection. LPA was greeted and allowed entry into the facility. LPA met with Administrator Associate (ADA)- Lisa Nodal. Entry way had setup for COVID 19 pre-screening.

LPA observed the inside and outside of the facility. LPA observed facility as being clean and free from clutter. Exits were free from obstructions. LPA observed clients and staff members interacting with one another.

LPA verified with ADA there is no medication administered or kept at the facility. LPA observed all chemicals and cleaning supplies locked and inaccessible to clients. LPA observed one knife in the facility as being locked and inaccessible to clients.

ADA stated clients bring their own food, and there is extra food in case clients need a snack or forgot their own food.

LPA observed fire extinguishers last services 11/1/2022. LPA observed water temperature in a bathroom as 105.8 degrees Fahrenheit. LPA observed first aid kits to be properly stocked with required items.

LPA observed CPR cards for staff members. LPA observed LIC610D on file and current. LPA observed facility's Mitigation Plan. LPA observed two staff member's files and two clients files.

No citations were issued and no deficiencies found at this time.

An exit interview was conducted, and a copy of this report was left with ADA Lisa Nodal, whose signature on

this form confirms receipt of these documents
