Facility Evaluation Report

Licensing Program Analyst (LPA) M. Flores arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit, and completed this annual visit with Erin Rosario, QIDP/RN. Licensee, Leah Nutter was not available; Administrator’s assistance provided a verbal approval for Erin Rosario to complete this annual visit. Several clients arrived later during the inspection.

LPA toured the building and observed fire exit of the facility to be clear and from obstruction. LPA toured the facility inside and out. The facility was observed to be at a comfortable temperature of degrees 71 degrees F, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Water temperature was checked in client’s bathroom and measured at 116.8 degrees F. Last disaster drill was completed on 10/25/23. LPA reviewed a sample of three staff and client's records. Smoke detectors and carbon monoxide are dual detectors, they were checked and operating. Fire extinguishers were charged and was serviced on 04/25/23.

LPA observed a 30-day supply of PPE. Medication observed and kept locked in a medication room. Cleaning supplies and chemicals stored and locked in a closet next to their main activity room.

There was outdoor seating for the residents. Outdoor area was clean and free of obstruction.

Exit interview was conducted and a copy of this report LIC809 was provided to Erin Rosario. The following documents are requested and submitted to Fresno CCL by Administrator. Forms requested: LIC308, LIC 309, LIC 400, LIC 402, LIC 500, LIC 610D, LIC 9282, and Administrator’s certificate will be submitted by 12/06/23
