Facility Evaluation Report

On 7/1/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred on 6/19/26, in which R1 had a half circle size quarter size scratch on the right arm/forearm. LPA met with Administrator (A1) Amanda Brewer. The scratch was noticed by R1’s mother. R1’s mother contacted the day program and asked if staff noticed the scratch. The Case Manager asked if staff noticed the scratch and S1 stated they noticed. S1 was not forthcoming in reporting to the day program and the R1’s mother. It was only reported after R1 was approached by Case Manager after R1’s mother called the day program regarding the scratch.

Administrator met with staff and issued a verbal warning about reporting incidents in a timely manner.

A citation is being issued on the attached 809-D for failure to report.

A copy of this report with Appeal Rights were issued to Administrator whose signature confirms receipt of this report.
