On 5/7/26 at 8:30 am Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Administrator (A1) Amanda Brewer and stated purpose of visit.

The department conducted interviews and reviewed records. Based on an incident report received by CCLD on 4/24/26 the Staff (S1) yelled over client while on duty. Per report the staff has received disciplinary action, where S1’s employment was terminated.

The following deficiencies is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6.

Exit interview conducted. A copy of this report was provided to Licensee whose signature confirms receipt of this report.
