Facility Evaluation Report

On 2/5/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up regarding details about statements that were made regarding a complaint made against the facility. LPA met with Administrator (A1) Amanda Brewer and stated the purpose of the visit. LPA interviewed staff about daily interactions with C1 while attending day program. The interviews were conducted based on concerns C1's family member had regarding if the staff is maintaining C1's Personal Rights. Based on interviews with staff, there were no findings to suggest that C1's Personal Rights were violated in any way.
