On 7/11/2023, Licensing Program Analyst(LPA) D. Ayers arrived unannounced to conduct a case management inspection.
The purpose of the inspection was to follow-up and gather information on an incident which occurred on 6/25/2023 and involved two residents, Resident 1(R1) and Resident 2(R2). During the visit, LPA toured the facility and interviewed the administrator and staff. No deficiencies were cited during the inspection. A copy of the reported was provided and exit interview conducted.
