Facility Evaluation Report
Licensing Program Analyst (LPA) Lissett Padgett arrived unannounced to conduct the Annual inspection. LPA met with Care Coordinator Luijean De Castro (CC) and explained the purpose of the visit. Facility was toured with CC.
During this visit, LPA toured the facility inside & out. Resident rooms contained required furnishings and lighting. LPA observed required items in bathrooms with hot water measuring between 105.1 to 114.3 degrees F. Resident hygiene supplies were properly stored and available.
The kitchen was toured observed in good repair with necessary items and appliances and sharps/knives were properly stored. LPA observed that refrigerator, was kept at a 41.4 degrees F, and the freezer at -4 degrees F, well maintained and clean. LPA observed a 2 day perishable food supply. The kitchen pantry was clean, organized, and had 7 days of non-perishable food. No expired food was observed. Medications are centrally stored and locked. Facility has designated visitation areas available inside and out.
Doors and passageways are unobstructed throughout the facility including outdoors. Perimeter fence is maintained locked.
First aid kit located in the medication room found to contain required items.
Fire Extinguishers are located throughout the facility and were serviced in 5/17/2024. Smoke and Carbon Monoxide detectors are tested annually with Johnson Controls Fire Inspection company. LPA reviewed and obtained a copy of the last inspection report conducted on 3/26/2024.
