Facility Evaluation Report
On 12/19/2023, Licensing Program Analyst (LPA) M. Flores arrived unannounced to continue the required annual inspection. LPA explained reason for inspection and was granted entry by staff. Licensee, Sundari Susan Kendakur approved for this LPA to complete this visit with Care Coordinator, Shannon Steele.
LPA checked water temperature in resident’s shared bathroom which read at 116.6 degrees F. LPA observed fire extinguisher and was last service on 6/09/2023. Knives are locked in the kitchen area. Fire extinguisher was charged and was serviced on 6/09/23.
During the visit a file review was conducted for residents and staff files, refer to 809D for deficiencies.
Exit interview was conducted with Care Coordinator Shannon Steele, whose signature confirms receipt of this report.
