The inspector’s account

On 4/28/26, Licensing Program Analyst (LPA) J. Leffall conducted an announced Pre-Licensing / Component III inspection. LPA introduced self, stated purpose of visit, and was allowed entry into the facility. LPA met with Administrator, Brandon Montelongo-Ayala. Facility has one central entry and exit. LPA and LPM observed a visitor log / temperature check upon entry.

The facility is a multi-small apartment facility that contains rooms for Assisted Living, Memory Care, Independent Living and fire clearance was granted for capacity of 148 residents.

LPA and toured the facility with Administrator. Common areas were furnished and had adequate seating and lighting available. Bedrooms had the required furnishings and are ready for occupancy. Hot water ranged from 116.7 to 118.9 degrees F in Memory Care, and 116.6 to 119.4 degrees F in Assisted Living. LPA and observed toilet area to have grab bars. Showers contained nonskid mats, non-skid floors, or non-skid strips. LPA and observed bed linens, and extra bed linens. LPA and observed hallway to have night lights. Apartments have an adequate supply of personal hygiene products. Kitchen was toured and observed to have clean dishes, plates, and utensils. Refrigerator temperature measured at 35 degrees F. Freezer temperature measured at -8 degrees F. Cleaning supplies and chemicals were observed in a locked in storage room. Medications are locked and secured in med-tech rooms both in Assisted Living and Memory Care. First aid kit was observed and contained all required items. A fire extinguisher was observed and has a service date of 10/10/25. Smoke detectors, carbon monoxide and fire sprinklers were observed operational per quarterly and annual reports from inspections courtesy of the State of California Fire Marshall provided to LPA.

Outside of facility toured. Exits were open and free of obstructions.

I have found that the applicants has met all pre-licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued.

Exit Interview conducted. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.
