Facility Evaluation Report

On 2/26/26 at 10:30 am Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Administrator (A1) Brandon Ayala-Montelongo and stated purpose of visit.

The department conducted interviews and reviewed records. The department followed up on information received regarding the facility's fire clearance and make-up air unit system. The department received the STD-850 along with a copy of the invoice payment to Green Earth Heating and Air of the repair of the Make-Up Air Unit system. The facility kitchen was experiencing malfunctions to the Make-Up Air Unit. A1 states the Fire Marshall recommended the Make-Up air unit be replaced. According to A1, Per Green Earth Heating, the Make-Up Air Unit will be repaired within 2 weeks. According to Green Earth Heating, the unit has been ordered and will enter the facility to replace in approximately 2 weeks.

Exit interview conducted. A copy of this report along with appeal rights was provided to Administrator whose signature confirms receipt of this report.
