On 12/26/25 at 9:00 am Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Administrator (A1) Brandon Ayala-Montelongo and stated purpose of visit.

The department conducted interviews and reviewed records. Based on information staff failed to generate a Unusual Incident Report within the required 7 day reporting period. The incident occurred on 11/11/25 and the Unusual Incident Report was received by the Department on 11/19/15.

The following deficiencies is being cited on the attached 809-D in accordance with California Code of Regulations, Title 22, Division 6.

Exit interview conducted. A copy of this report along with appeal rights was provided to Administrator whose signature confirms receipt of this report.
