Facility Evaluation Report
On 11/24/25, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred on 11/10/25. LPA met with Administrator Brandon Ayala-Montelongo.
On 11/10/25 it was reported that RA did not help R1 after shower was finished. R1 called RA 4 times and RA stated she was on the phone sitting in R1's chair. LPA asked A1 what disciplinary action will be taken or has been taken. A1 stated facility was planning on terminating RA on 11/18/25, however RA resigned as of 11/18/25 before facility was able to terminate RA. HR was conducting an internal investigation before R1 resigned..
A citation is being issued on the attached 809-D in accordance with California Code of Regulations, Title 22,Division 6.
A copy of this report with Appeal Rights was issued to Administrator whose signature confirms receipt of this report.
