On 12/1/25, Licensing Program Analyst (LPA) J. Leffall met with Administrator (A1) Brandon Ayala-Montelongo to conduct a Case Management. The purpose the visit is to conduct the complaint investigation from a Co-Complaint received on 11/25/25. The complaint is to investigate if Staff (S1) is intoxicated while on duty. LPA interviewed Administrator, and Residents.
At this time this complaint needs further investigation.
