Facility Evaluation Report

On 4/24/25, Licensing Program Analyst J. Leffall and Licensing Program Manager S. Moua conducted a POC case management inspection to obtain details for repeat citation 87303(a) – Maintenance and Operations issued on 4/11/2025. Civil penalty for repeat citation was not issued on 4/11/2025 and will be issued today.

LPA and LPM met with Administrator (A1) Michelle Ramos and stated the purpose of the visit. The dining room area and door was toured together.

The following details regarding the facility’s POC was obtained:

-Administrator stated and maintenance staff Eric confirmed that the facility has 9 sandbags that will be placed out to mitigate any immediate rain

-Maintenance is adding additional weather stripping to the door

Details regarding the facility POC plan was requested – .

-An independent/outside agency assessment (Blue Sky) was contacted for repairs

-Date of the assessment, quote, and contact information is to be provided as part of the POC

-Estimation of when the repairs will be completed

Civil penalty form (LIC 421-FC) was completed and signed on today’s date and provided to the Administrator.

Administrator was informed that if a POC is not submitted to the Department by the due date, ongoing civil penalty for failure to correct will be assessed.

A copy of this report was issued to A1 whose signature confirms receipt of this report.
