Facility Evaluation Report

Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility to verify that staff (S1) has enrolled in an anger management class. Per the Stipulation, CDSS Case No. 7824250002, S1 must enroll in an anger management class by September 14, 2025.

LPA arrived at the home 9/8/2025 at 3:10 an no one was at the facility. LPA left a card on the door and waited until about 3:30 and began to leave the neighborhood. As LPA was leaving, facility van was seen coming down the street. LPA returned to the facility.

LPA met with staff Shanika Cooper and they then called Administrator (AD) Naomi Medina, to assist. AD arrived at the home a short time later. LPA explained the reason for the visit on this day and AD contacted S1 and was able to provide requested documentation.

LPA was provided with enrollment verification from Anger Masters, enrollment date of August 1, 2025 for S1.

A copy of this report was provided to the Administrator, Naomi Medina, whose signature confirms receipt.
