On 5/31/2024, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA introduced self, stated purpose of visit and allowed entrance. LPA met with Administrator, Constance Peters and stated purpose of visit.
Currently 46 residents in care. There are four residents on hospice, and six residents currently receiving home health services.
Outside of facility toured. All exits open free of obstruction. No hazards observed
No deficiencies observed or cited during visit.
